Before You begin participating in Mixed Martial Arts
Modern Combat Academy (“Modern Combat Academy, We, Us or Our”) is a full-service mixed martial arts academy offering instruction in Brazilian Jiu-Jitsu, Kickboxing, MMA, Boxing, Wrestling, Judo, and Muay Thai, as well as supplementary fitness programs to support your training (together, “Mixed Martial Arts”).
Members (including parents or guardians who have authorised their child to participate in Mixed Martial Arts at Modern Combat Academy) (“You or Your”)
a. You agree to complete the health questionnaire included in this Agreement and to accurately report all health matters that may affect Your ability to train safely.
b. You agree to provide Us with updated health information if Your health deteriorates or changes adversely.
c. You acknowledge the likelihood of a physical/health condition You may have of which You are not aware may be aggravated or worsened by participation in Modern Combat Academy.
d. Modern Combat Academy strongly recommends You:
I. seek your own independent medical advice before commencing training at Modern Combat Academy; and
II. Seek medical advice if You become concerned about Your health at any point during Your Membership.
e. You agree to follow Modern Combat Academy instructions regarding the use of protective equipment as instructed, including but not limited to:
· Mandatory use of mouthguards at all times
· Shin Pads
· Head Gear
· Groin Guards
· No Jewelry to be worn during martial arts activities
Failure to do so may result in an inability to participate in class and/or the termination of Your membership.
f. You agree to always follow the instructions of Modern Combat Academy instructors while within Modern Combat Academy facility.
g. Should an incident/injury occur, you agree to promptly report the injury at the time it happens and participate in the Modern Combat Academy Incident Reporting process.
Emergency Medical Assistance
a. In the event of an emergency, You acknowledge and agree that We may administer all necessary first aid and call emergency services on Your behalf.
I. You acknowledge and accept that there may be no or inadequate facilities for transport or treatment.
b. You agree to reimburse Us all costs associated with emergency services called on Your behalf.
If, because of any religious objection to emergency or medical treatment, You do not want Us to treat Your injuries, You must:
I. notify Us as soon as possible of this;
II. at the same time, provide Us with written instructions informing Us how You would like Us to deal with Your injury and who You want Us to contact; and
III. if You are injured, tell Us (if You are able), that You have an objection to medical treatment, and We have Your instructions.
Photography Release
You acknowledge that when in the gym, media may be created for advertisement and social media use. You acknowledge and authorise your likeness and make no claim to any copyright of these images/videos.
